Individual Support Services (ISS) Renewal Process

All individuals who are enrolled in ISS and will continue with the program in 2021 will need a renewal packet completed. All packet materials are to be completed by the Care Coordinator and must be returned to Mozaic by 10/31/20 in order for the ISS coordinator to compile the packets and turn into OPWDD by the November 15th deadline.
